Lead Organizations
Who are the lead agencies and what do they do?
The SC Roadmap is guided by four lead agencies that bring together expertise in community engagement, policy, research and evaluation, and implementation science. Working collaboratively, these partners serve as the backbone organizations that coordinate statewide efforts to improve the nonmedical factors that impact health. Together, they ensure the Roadmap stays grounded in community voice, informed by data, aligned with policy opportunities, and supported by sustainable systems that drive lasting change across South Carolina.
Center for Community Health Alignment (CCHA)
Administrative Lead
The Administrative Lead (Admin) supports implementation by engaging program partners, communicating the established Common Agenda Pathways and Strategies, and facilitating connections and alignment of efforts addressing nonmedical factors impacting health within communities and across the state. This lead serves as the primary convener of the Community Voice Advisory Board, Governing Council, and other backbone leads. In addition, the Administrative Lead partners with other leads to secure funding that supports Common Agenda strategies and actions.

South Carolina Hospital Association (SCHA)
Programmatic Lead
The Programmatic Lead drives the implementation, spread, and alignment of innovations, practices, and processes related to nonmedical factors impacting health at the regional and local levels. This lead informs innovations that build or enhance local capacity to address nonmedical factors impacting health based on community needs through multi-sectoral collaboration, particularly between community and healthcare organizations. Capacity building includes providing technical assistance and training that supports integration of processes and supports focused on nonmedical factors into existing programs and systems.

South Carolina Institute of Medicine and Public Health (IMPH)
Policy Lead
The Policy Lead is responsible for identifying evidence-based policy and system changes that are proven to help impact health and have a positive financial return. The Policy Lead helps community leaders and decision-makers understand policies that address nonmedical factors and how they can improve health while saving money. This lead equips community leaders and decision-makers with the evidence and insight needed to advance policies in areas such as food access and housing stability while ensuring these strategies align with the Roadmap’s Common Agenda goals to create meaningful impact at both state and local levels.

Furman University Institute for the Advancement of Community Health (IACH)
Research and Information Lead
The Research and Information lead, in partnership with the other SC Roadmap organizational leads, supports the development of the Common Agenda, with an emphasis on the Technology and Infrastructure Pathway. The IACH also conducts research on potential technology and infrastructure innovations that could advance care for nonmedical factors impacting health like food insecurity and housing. The IACH will also play a key role in facilitating dialogue and potential partnerships to advance action around Technology and Infrastructure Pathway strategies. The IACH also leads the process evaluation for the SC Roadmap, supports the other leads in the submission of SC Roadmap grant applications, and leads the development of research and evaluation plans for projects that are successfully funded as part of the common agenda.
