Why this matters
Even though they want to support people with needs like food, transportation or housing, healthcare and community organizations often lack aligned approaches that could help them better address these (and other) nonmedical factors that impact health. Strengthening coordination around screening, referrals, and follow-up enables more effective service connections and improved outcomes. This work also requires stronger partnerships with non-traditional sectors that influence these conditions.
Purpose
Strengthen the capacity of healthcare providers, community organizations, and cross-sector partners to assess their organizational readiness and implement effective approaches to address nonmedical factors impacting health (NFIH).

Objective 1.1: Learn and document successful processes and partnerships that support healthcare and community partners in addressing NFIH and replicate these approaches through training and technical assistance to improve coordination and outcomes across South Carolina
Activity 1.1.1: Conduct an updated statewide assessment to understand current practices, challenges and opportunities and disseminate what is learned.
Activity 1.1.2 Implement and evaluate a NFIH Community of Practice
